The Tenant hereby grants permission to the Landlord, its Agents, and/or Sub-Contractors to Enter the Apartment to make the necessary Requested Repairs if I am not present.
* Please indicate if the Maintenance Request can be completed in a timely manner or if it is an urgent matter and requires immediate attention.

For example: Clogged Toilets or Lock outs are not considered urgent matters. A burst pipe, a flood or running water is considered urgent.
